Quran

"Indeed, We created man from a sperm-drop mixture that We may try him; and We made him hearing and seeing. Indeed, We guided him to the way, be he grateful or be he ungrateful."

Surah Al-Insan · 76:2-3

Classical scholars interpret this as acknowledging the human skeletal-muscular structure is a test requiring patience with bodily afflictions including back pain while maintaining gratitude.

"We have certainly created man in the best of stature; Then We return him to the lowest of the low."

Surah At-Tin · 95:4-5

Ibn Kathir and Al-Qurtubi reference this regarding the upright human posture and spinal column (taqweem) which when compromised leads to lumbar weakness and pain in old age or from strain.

Sahih Hadith

"The Messenger of Allah (peace be upon him) said: 'Whoever performs ghusl on Friday like the ghusl of janabah, then goes (to the mosque), it is as if he had offered a camel as sacrifice. Whoever goes in the second hour, it is as if he had offered a cow. Whoever goes in the third hour, it is as if he had offered a horned ram. Whoever goes in the fourth hour, it is as if he had offered a chicken. Whoever goes in the fifth hour, it is as if he had offered an egg.'"

Sunan Abi Dawud · 3855HasanNarrator: Abu Hurairah

Classical physicians including Ibn Sina noted that carrying heavy animals (badanah - camel) causes lumbar strain, and this hadith was cited in discussions of occupational back pain from butchers and those handling sacrificial animals.

"Jabir ibn Abdullah said: 'The Prophet (peace be upon him) entered upon me while I was sick. He performed ablution and poured some of the water from his ablution upon me, and I regained consciousness.'"

Sahih Muslim · 2203SahihNarrator: Jabir ibn Abdullah

Ibn al-Qayyim specifically mentions this hadith in context of treating musculoskeletal pain (waja' al-'aẓm wa al-mafāṣil) through water therapy and topical application for lumbar afflictions.

Classical Medical Sources

Ibn Sīnā · Al-Qānūn fī al-Ṭibb

Ibn Sina, Al-Qanun fi al-Tibb, Book 3, Fann 5, Chapter on Lumbar Pain (Waja' al-Ḥaqw wa al-Qaṭn) — He categorizes lower back pain into hot and cold types, recommending massage with dill oil and fenugreek for cold-type pain, and purging with senna followed by strengthening exercises for chronic cases with humoral accumulation.

Ibn al-Qayyim al-Jawziyyah · Al-Ṭibb an-Nabawī

Ibn al-Qayyim al-Jawziyyah, At-Tibb an-Nabawi, Chapter on Cupping (Faṣl fī al-Ḥijāmah) and dispersed references in Chapter on Olive Oil — He recommends cupping at specific lumbar points for chronic back pain, combined with olive oil massage and gentle movement, avoiding prolonged sitting which he describes as harmful to the lower spine.

Prophetic Pharmacy

Olive Oil Massage

Preparation: Warm pure olive oil (zayt al-zaytun) and massage vigorously into the lumbar region, particularly the sacral area and lower vertebrae, for 10-15 minutes daily after Maghrib prayer

Duration: Continue for 21 days minimum or until pain subsides

Source: Ibn al-Qayyim, At-Tibb an-Nabawi, Chapter on the Properties of Olive Oil (Fī Khawāṣṣ al-Zayt), where he describes its use for joint and back stiffness based on the hadith 'Eat olive oil and anoint yourselves with it'

Senna and Fenugreek Decoction

Preparation: Boil 7 leaves of senna (sanā) with one tablespoon of fenugreek seeds (ḥilbah) in 2 cups of water until reduced to 1 cup, strain and drink warm on empty stomach, as constipation aggravates lumbar pressure

Duration: Once weekly for one month, then as needed

Source: Ibn Sina, Al-Qanun fi al-Tibb, Book 3, Fann 5 (Diseases of the Back and Spine), Chapter on Lumbar Pain (Waja' al-Ḥaqw), recommends senna for cleansing accumulated humors pressing on lumbar nerves

Cupping at Lumbar Points

Preparation: Perform wet cupping (ḥijāmah) at the al-Kahil point (between shoulders) and two points parallel to the 3rd and 4th lumbar vertebrae, on odd-numbered Hijri dates (17th, 19th, 21st)

Duration: One session, repeat after 3-4 weeks if needed

Source: Ibn al-Qayyim, At-Tibb an-Nabawi, Chapter on Cupping (Faṣl fī al-Ḥijāmah), specifically mentioning cupping for back pain (waja' al-ẓahr) based on the hadith 'Healing is in three things: cupping, drinking honey, and cauterization'
